Frozen shoulder occurs when the capsule surrounding the shoulder joint becomes inflamed, thickened, and contracted. This leads to progressive loss of movement in all directions — raising the arm, reaching behind the back, and rotating the shoulder all become difficult and painful.
The condition typically progresses through three stages: Freezing (increasing pain and stiffness, 2–9 months), Frozen (less pain but maximum stiffness, 4–12 months), and Thawing (gradual return of movement, 5–24 months). Physiotherapy is effective at every stage and can significantly reduce the total duration.
Treatment is tailored to your current stage of frozen shoulder. In the freezing stage, the focus is on pain management — gentle mobilisation, electrotherapy, and pain-free range-of-motion exercises. In the frozen stage, we gradually increase manual mobilisation intensity and stretching to regain movement. In the thawing stage, strengthening exercises are added to restore full function.
Sessions are typically 2–3 times per week, with daily home stretching and exercises essential for progress. Manual therapy techniques include Maitland mobilisation, Mulligan mobilisation with movement, and capsular stretching. Treatment duration varies: mild cases may resolve in 2–3 months, while severe cases may take 6–9 months.
